Responsibilities
- Perform clinical assessments to identify patient needs and determine appropriateness for physical therapy; develop and revise Plan of Care in collaboration with the physician and interdisciplinary team.
- Document, review, and complete Plan of Care (POC), progress notes, and other clinical records; adjust the POC based on ongoing evaluation data.
- Plan and deliver treatment for neuromuscular, neurological, and orthopedic conditions in alignment with medical referrals and therapy assessments.
- Instruct patients and families on safe transfer techniques and the use of equipment such as walkers, wheelchairs, canes, braces, and prosthetic devices; select and adapt therapy equipment to patient needs and home environments.
- Provide patient and family education tailored to age, condition, culture, and learning readiness; ensure documentation of education provided.
- Collaborate with the interdisciplinary team; supervise or coordinate other staff as applicable; apply infection control and universal precautions in daily care.

Requirements
- Active license as a Physical Therapist in the state of Washington – Required
- Proof of current CPR certification – Required
- Current driver's license; reliable transportation and valid auto insurance – Required
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program – Required
- 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting – Required
- Previous home care experience – Preferred
