Responsibilities
- Conduct comprehensive PT evaluations for residents and develop individualized treatment plans tailored to mobility, balance, strength, and activities of daily living.
- Implement interventions including therapeutic exercises, gait training, transfers, balance activities, and functional mobility training; adapt as goals progress.
- Reassess progress regularly, document outcomes in the electronic health record, and communicate findings to the interdisciplinary team to inform care planning.
- Educate residents and families on home exercise programs, fall prevention, and safety during mobility; participate in discharge planning and transitions of care when appropriate.
- Ensure patient safety, infection control, and compliance with facility policies, state regulations, and privacy guidelines.

Requirements
- Active California Physical Therapist license (or eligibility to practice in California)
- Doctor of Physical Therapy (DPT) degree
- CPR/BLS certification
- Minimum 1 year of clinical PT experience; geriatrics or skilled nursing setting preferred
- Ability to work in a skilled nursing facility setting and participate in travel assignment logistics
