Responsibilities
- Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations; develop individualized treatment plans to restore or improve function.
- Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of modalities as ordered by the physician.
- Monitor resident progress and adjust care plans to ensure treatment goals are met; participate in discharge planning as appropriate.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements; accurately document sessions, progress notes, and billing information; ensure compliance with state, federal, and facility standards.

Requirements
- Active Physical Therapist license in the state of practice (Missouri)
- Graduate of an accredited Physical Therapy program
- Experience in SNF or geriatric rehabilitation preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently and as part of an interdisciplinary team
- Ability to work in a Skilled Nursing Facility/Long-Term Care setting
