Responsibilities
- Perform initial PT evaluations and develop individualized treatment plans for residents
- Implement therapeutic exercises to improve mobility, strength, balance, and functional independence
- Conduct gait training, transfer training, and rehabilitation for gait, posture, and functional activities
- Monitor progress, document outcomes, and adjust treatment plans accordingly
- Collaborate with an interdisciplinary care team to support discharge planning and transitions of care
- Ensure accurate and compliant documentation and adherence to facility policies and standards

Requirements
- Experience delivering physical therapy services in geriatric or skilled nursing settings
- Ability to work on-site for 35 hours per week
- Availability to begin on 07/13/2026 and fulfill a 13-week assignment
- Comfortable with a daytime schedule of 08:00–16:30
- Strong communication and documentation skills to support interdisciplinary care
