Responsibilities
- Conduct initial physical therapy evaluations for residents in a long-term care setting and develop individualized treatment plans.
- Implement therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
- Monitor progress, document outcomes, and adjust treatment plans as needed.
- Collaborate with an interdisciplinary care team to coordinate patient care and communicate progress.
- Ensure safety during therapy sessions, adhere to infection control practices, and maintain accurate documentation.
- Educate residents and families on exercise programs and discharge planning as appropriate.
Requirements
- Availability to work 38 guaranteed hours per week.
- Ability to work in a long-term care/nursing home environment.
- Willingness to participate in a 13-week travel assignment.
- Strong communication, documentation, and teamwork skills.
- Flexible, with the ability to adapt to a post-acute care setting and follow facility protocols.
- Commitment to patient-centered care and safety in all therapy activities.
