Responsibilities
- Perform comprehensive outpatient physical therapy evaluations and formulate individualized treatment plans
- Implement evidence-based therapeutic interventions, including therapeutic exercise, manual therapy, and modality use as appropriate
- Monitor progress, reassess goals, and adjust treatment plans to optimize functional outcomes
- Document treatments and outcomes accurately in the patient record; ensure timely communication with the care team
- Educate patients and families on home exercise programs, safety, and follow-up plans; promote adherence and safety

Requirements
- Education: Bachelor’s Degree from a four-year accredited college or university in physical therapy (or a related field)
- Licensure: California Physical Therapy license (active)
- Certification: AHA BLS Certification
- Professional preference: APTA membership preferred
- Experience: Outpatient physical therapy experience preferred; must be able to work in an outpatient setting and comply with facility policies
