Responsibilities
- Conduct initial and ongoing PT evaluations, develop evidence-based plans of care, and implement therapy to support residents’ functional goals.
- Provide one-on-one and group therapy sessions, document progress, and adjust treatment plans based on resident response.
- Administer therapy modalities, assistive devices, and therapeutic activities appropriate for geriatric rehabilitation; maintain accurate care records.
- Collaborate with an interdisciplinary team to coordinate care, participate in goal setting, and support discharge planning and home exercise programs.
- Educate residents and families on exercise techniques, safety, fall prevention, and carryover strategies to support continued recovery.
- Ensure patient safety, adhere to infection control practices, and comply with facility policies and regulatory requirements during therapy sessions.

Requirements
- Current Physical Therapist license in North Carolina or eligible for licensure
- Doctor of Physical Therapy degree from an accredited program
- Relevant experience in skilled nursing facility or geriatric rehabilitation preferred
- Ability to work 35 hours per week in a travel assignment; reliable transportation and scheduling flexibility
- Strong documentation, communication, and teamwork skills
- Ability to work in a skilled nursing facility setting and follow professional standards
