Responsibilities
- Perform comprehensive PT evaluations for residents and develop individualized treatment plans aligned with geriatric rehabilitation needs.
- Deliver evidence-based physical therapy interventions aimed at improving mobility, balance, strength, pain management, and activities of daily living.
- Monitor patient progress, adjust treatment plans, and document outcomes in the medical record per regulatory requirements.
- Collaborate with an interdisciplinary team to support resident goals, discharge planning, and ongoing care coordination.
- Ensure patient safety during therapeutic activities, implement gait and transfer training, and apply assistive devices as indicated.
- Maintain compliance with licensing, facility policies, and applicable state and federal regulations related to physical therapy practice in a long-term care setting.

Requirements
- Active Physical Therapist license to practice in the United States or eligibility for licensure in West Virginia.
- Ability to work in a long-term care/geriatrics setting with a diverse resident population.
- Availability to begin on 05/18/2026 and commit to a 13-week assignment with guaranteed hours.
- Experience in geriatric physical therapy or a long-term care environment is preferred.
- Strong clinical documentation skills and teamwork mindset to collaborate with an interdisciplinary care team.
