Responsibilities
- Conduct comprehensive physical therapy evaluations for residents in the skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and activities of daily living.
- Provide therapeutic exercises, gait training, balance activities, manual therapy, and modalities as appropriate.
- Monitor patient progress, document outcomes, and adjust treatment plans to optimize functional goals.
- Collaborate with an interdisciplinary team (nursing, therapy colleagues, social work, physicians) to support discharge planning and ongoing care.
- Ensure patient safety, maintain infection control standards, and complete timely, compliant documentation.

Requirements
- Active physical therapy license in Maine (or eligible to practice in the state)
- Doctor of Physical Therapy (DPT) degree or equivalent credential
- Ability to provide direct, in-person therapy in a skilled nursing facility setting
- Willingness to undertake a travel assignment for 13 weeks starting 06/01/2026
- Strong communication and documentation skills; ability to work effectively within a multidisciplinary team
